ABOUT MARK HAIM
Mark Haim has been choreographing for over 35 years. He has created and restaged dances for companies such as the Nederlands Dans Theater, Ballet Frankfurt, the Limon Dance Company, The Joffrey Ballet, the Bat-Dor Dance Company of Israel, and the Juilliard Dance Ensemble. His full evening solo project, The Goldberg Variations, premiered at the ADF, and performed at Danspace Project, The Kennedy Center, On The Boards and other venues in the U.S, Europe, and Asia. This Land Is Your Land, opened the ArtDanThe Festival in Paris, and was seen at the Joyce Theater in NYC, the Nasher Museum of Art  and the National Performing Arts Center in Beijing. Mark was on the faculty of the American Dance Festival 1993-2015 and has taught at universities throughout the US, companies in Belgium, Holland, Germany, Italy, Portugal, Poland, Latvia, Russia, Argentina, Chile and Japan. He currently teaches at Dance Fremont, The International Ballet Academy, Spectrum Dance and Pacific Northwest Ballet.

STG PARTICIPANT ASSUMPTION OF RISK AND RELEASE OF LIABILITY

I recognize that risk of illness and/or injury may be inherent in any dance or onstage performance; however, the above individual is participating with the express agreement of, and understanding that Seattle Theatre Group and its directors, officers, employees and agents are hereby released from any and all claims, costs, liabilities, expenses, or judgments associated with this training program. I hereby execute and deliver this Waiver and Release form to induce Seattle Theatre Group and All That Dance to permit participation in the program indicated above.

I hereby grant the Seattle Theatre Group staff permission to administer first aid and/or call 911 in the case of a medical emergency while I/my child is attending classes, rehearsals, and performances. I realize and understand that Seattle Theatre Group will attempt to notify parents and/or guardians in case of an emergency.

PHOTO RELEASE

I hereby understand that Seattle Theatre Group may take photographs, volumetric video for 3D AR/XR recording and digital distribution, and/or video of the participant indicated in this form, and I authorize Seattle Theatre Group to use these photographs and video for archival and publicity purposes.
